Compliance Statement (HB 1824) 

This form must be presented to ALL Washington state schools that your club uses for practices and tournaments, along with a certificate of insurance, in order to be in compliance with the Lystedt Law concerning head-injury/concussion information.

Incident Report Form

Form that is used when there is an injury or property damage that occurs at a practice or sanctioned tournament.  The coach or tournament director MUST fill this out for any injury
and SUBMIT it to the region office AS SOON AS POSSIBLE.

Tournament Feedback Form ~ Tournament Directors

This form is to be filled out by Tournament Directors as a self-evaluation tool , as well as a tool to document anything else deemed notable (information about teams/day officials/coaches/facilities, etc).  Please fill out the form completely and send to the region office.
