This form must be presented to ALL Washington state schools that your club uses for practices and tournaments, along with a certificate of insurance, in order to be in compliance with the Lystedt Law concerning head-injury/concussion information. |
Incident Report Form |
Form that is used when there is an injury or property damage that occurs at a practice or sanctioned tournament. The coach or tournament director MUST fill this out for any injury and SUBMIT it to the region office AS SOON AS POSSIBLE. |
