2009-2010 Tryout Information

Tryout Form
This form must be presented at each tryout. **As required by the State of Washington, a second page has been added that contains IMPORTANT concussion/head injury information. This MUST be read by both player and parent and your signature serves as acknowledgement that you have been informed of the risks and proper response information.

**If you did not make a team and live in the Spokane area, there will be an "all club/all ages tryout"
December 6 at University H.S. from 10 am - 1 pm.
