Evergreen Region Forms

ATTENTION CLUB DIRECTORS!
The following list contains all forms for the 2009-2010 season. Some forms will be retained by the CLUBS, while the following will still be required to be sent to the office:

Bylaws
The current Bylaws for the Evergreen Region Volleyball Association.
Chaperone Responsibilities Form
Form that MUST be filled out by all registered chaperones and sent to the region office with all chaperones' registrations
Club Directors' Checklist/Guide
A "cheat sheet" of sorts that assists a club director in getting ready for the upcoming season THIS IS A GREAT TOOL FOR ALL CLUB DIRECTORS!
Coaches' Code of Ethics
Code of ethics to be followed by all Evergreen region coaches. A copy must be signed and sent to the region office with all coaches' registrations
**NOTE: Coaches must also read the Concussion Fact Sheet for Coaches and view the Concussion Video prior to signing!**
Compliance Statement (HB 1824) **NEW!**
This form must be presented to ALL Washington state schools that your club uses for practices and tournaments, along with a certificate of insurance, in order to be in compliance with the new Lystedt Law concerning head-injury/concussion information
Concussion Fact Sheet for Coaches**NEW!**
This form must be read by ALL coaches in order to be in compliance with the new Lystedt Law concerning
head-injury/concussion information
Concussion Form for Players/Parents**NEW!**
This form must be read and signed by ALL players and parents in order to be in compliance with the new Lystedt Law concerning
head-injury/concussion information. THIS FORM MUST BE TURNED IN TO YOUR CLUB DIRECTOR!!!
Concussion Video **NEW!**
This video must be viewed by ALL coaches in order to be in compliance with the new Lystedt Law concerning
head-injury/concussion information
Incident Report Form
Form that is used when there is an injury or property damage that occurs at a practice or sanctioned tournament
Individual Membership Form

All registration for the season will be done ONLINE, but this form will be required to be submitted to each club for their records.

Letter of Intent Form
Binding agreement form between players, parents, and clubs
Medical History & Release Form
Form that contains medical information for each player and MUST be carried by the coach to all training and competition

Tournament Feedback Form-Tournament Directors
This form is filled out by TOURNAMENT DIRECTORS as a self-evaulation tool as well as a tool to document anything else that a tournament director may deem notable (information about teams/day officials/coaches, facilities, etc.). Please fill out the form COMPLETELY, and send to the region office.
